Chapter 770 Wisconsin State Statues
One or both domestic partners may terminate the domestic partnership by filing a completed Notice of Termination of Domestic Partnership form within the county where the Declaration of Domestic Partnership was issued. The notice must be signed by one or both domestic partners and notarized. Identification and the certified copy of the Declaration of Domestic Partnership must be presented.
If only one of the domestic partners is signing, that individual must also file an Affidavit of Notification of Intent to Terminate a Domestic Partnership stating either of the following:
- That the other domestic partner has been served in writing (Summons):

Provide notice by publication in a newspaper of general circulation in the county in which the residence most recently shared by the domestic partners is located. The notice need not be published more than one time. The filing partner must post the notice 20 days prior to submitting the Affidavit of Notification of Intent to Terminate a Domestic Partnership to the County Clerk. A sample of the published notice is available from the County Clerk.
The Affidavit of Notification of Intent to Terminate a Domestic Partnership form can be picked up in the County Clerk's Office.
FEE
The fee for a Termination of Domestic Partnership is $100.00 CASH ONLY.
FILING OF THE CERTIFICATE
The domestic partner(s) shall submit the Certificate of Termination of Domestic Partnership to
the Register of Deeds of the county in which the declaration of Domestic Partnership is recorded. The Register of Deeds shall record the certificate and forward the original to the state registrar
of vital statistics.
The termination of a domestic partnership is effective 90 days after the Certificate of Termination of Domestic Partnership is recorded at the Register of Deeds office.
If a party to a domestic partnership enters into a marriage that is recognized as valid in this state, the domestic partnership is automatically terminated on the date of the marriage.
