PASSPORT INFORMATION

OFFICE HOURS
Applications for passports are processed Monday – Friday between the hours of 8:00 a.m. – 4:00 p.m. at the Racine County Clerk’s office, 1st Floor, north end of the building.

THE FOLLOWING INFORMATION IS REQUIRED:
You will need a current application form (Form DS-11) properly completed. Forms can be obtained from the County Clerk’s Office or by visiting the website at travel.state.gov. You can fill the form out and bring it in. Please do not sign the form until requested to do so in the presence of an acceptance agent. Forms must be filled out in black ink only.
Proof of Citizenship: A certified birth certificate must include your given name and surname, date and place of birth, full name of parents, date the birth record was filed, and the seal or other certification of the official custodian of such records. Naturalization and consular report of birth abroad are considered proof of citizenship.
Proof of Identity: A valid driver’s license or picture ID.
Photograph: A passport photo following the Passport Agency specifications. Your picture can be taken in the County Clerk’s Office for a fee of $17.00.
FEES FOR PASSPORT BOOK
A check or money order only made out to U.S. Department of State. Check the travel.state.gov website or call our office for amount.
There is a separate processing fee, payable to the Racine County Clerk’s office in cash, check, or money order. We do offer 1-2 day delivery to the Passport Agency for an additional fee, if requested.
All applicants must appear in person. If the applicant is 15 years or younger, both parents are required to appear along with their proper identification. If applicant is 16 or 17 years old, please visit travel.state.gov for more information. To prevent processing delays, it is highly recommended one parent is present and can provide proper identification.
